Multivariate Analysis of the Factors for the Recurrence of Anorectal Abscess



Abstract
During the period between 1981 and 1990, 160 patients underwent surgical treatment at the National Medical Center for the anorectal abscesses which were regarded as nonspecific cryptoglandulra infection. A retrospective analysis of these patients
defined the biologic behavior, surgical management predictive factors influencing recurrence of anorectal abscess. The follow up was possible in the all cases except two, and the average follow up period was 10.7 months (SE 1.9 months, range 1~60
months). The male and the ages of the third and fourth decades had the most frequent incidence. There were a 50.6% incidence in the ischiorectal absess, 37.5% in the perianal, 8.1% in the supralevator, 3.8% in the submucosal. The thirty five
percent of
the patients have had recurrences such as abscess(11.8%), fistula(20.3%), abscess and fistula(3.9%). The deeply-seated abscesses were more likely to recur than the superficiallyseated abscesses(p=0.084). the single colonic aeronic bacteria, or
the
mixed
infecion of the single colonic aerobe and anaerobe was the most frequent cultured organism. There was no cases of recurrence among the abscesses from which bacteria were cultured. The method of treatment(p=0.007) and the period(p=0.032) between
the
onset of symptom and the surgery were the significant prognostic factors of the recurrence by multivatiate analysis of stepwise logistic regression. The anatomic location of the abscesses, the kind of bacteria, the past history showed the
significant
impact on the kind of recurrence(i. e. abscess and fistula) in the recurred patients by the discriminant analysis. Thus, with the information of the anatomy of the abscess, of the kind of the bacteria and of the past history the early proper
treatment
may improve the cure of the anorectal abscesses.
